How to make a referral

We understand that taking the first step can be overwhelming. That’s why our referral process is simple, supportive, and responsive. Whether you're a professional, parent, or carer – we're here to help.

First Steps

Add a description about this category

Make an Enquiry

Start by contacting us via our online referral form, email, or phone. A member of our team will respond promptly to answer questions and provide guidance on next steps and to decide what type of service it is that you need.

Submit a Referral Form

We’ll send you a referral pack or guide you through the online submission. This includes key information about the children, family circumstances, and the type of service you’re seeking (e.g., contact, assessment, parenting support).

Initial Assessment and Screening

Our experienced team will review your referral and carry out a preliminary risk assessment to ensure we can meet the needs safely and effectively. Where appropriate, we may request additional details or documents.


Next Steps

Initial Meetings

We’ll invite all parties for initial meetings, including a child-centred induction where suitable. These sessions help us understand each family’s needs and build rapport before contact or assessments begin.

Tailored Support Plan

Once agreements are in place, we’ll provide a clear plan, schedule dates, and begin delivering the service. You’ll receive regular updates and professional reports as needed.


We can regularly review the plan to make sure it is what you need. 

Begin Family Time

With the plan in place, contact will commence at Nexusl Family Connect or in the community. Our dedicated staff, will ensure a safe and neutral space for positive interactions between children and non-residential parents.
